Registered Nurse (RN) Case Manager Ascend Home Health & Hospice Full-TimePosition Overview Ascend Home Health & Hospice is seeking a compassionate and skilled Registered Nurse (RN) Case Manager to join our team in Preston, ID. In this role, you will provide high-quality, patient-centered care while coordinating services to support patients and their families throughout their healthcare journey. You'll play a key role in ensuring continuity of care, improving outcomes, and making a meaningful impact in the lives of those we serve. Key Responsibilities Provide comprehensive nursing care to patients in their homes and/or assisted living settings Conduct initial and ongoing patient assessments and develop individualized care plans Coordinate care with physicians, therapists, social workers, and other team members Monitor patient progress and adjust care plans as needed Educate patients and families on diagnoses, medications, and care plans Complete accurate and timely documentation in accordance with regulatory standards Participate in on-call rotation as needed Qualifications Current, active RN license in the state of Idaho (or compact license if applicable) Strong clinical assessment and critical thinking skills Excellent communication and organizational abilities Ability to work independently while collaborating with a team Valid driver's license and reliable transportation Schedule & Benefits Full-time position Competitive salary (based on experience) Comprehensive benefits package including: Medical, Dental, and Vision Insurance Generous PTO program 401(k) with employer match Why Join Ascend?
