License: Current and valid Registered Nurse License Certifications: Certification in Diabetes Education Must-Have: Bachelor of Science in Nursing Description: The Registered Nurse (RN) plays a pivotal role in the holistic management and education of patients with diabetes. The RN understands the complexities of diabetes management from medication familiarity to technical troubleshooting of devices. They ensure that patients receive optimal and personalized care in collaboration with the entire diabetes care team. The RN will possess comprehensive knowledge of all diabetes medications to counsel patients on proper administration techniques, potential side effects, and expected outcomes. They will prepare prior authorizations for diabetes-related medications for insurance companies. The RN will lead the process to appeal denials of medications to ensure timely access to vital treatments. They will work with pharmacies to solve coverage issues so patients have safe and effective substitutions of medications where appropriate. The RN will educate patients on the fundamentals of diabetes, ensuring a comprehensive understanding of the disease from both a biological and practical management perspective. They will foster patient-centered approaches, tailor teaching methods to individual needs, and ensure patients understand the intricacies of managing their condition. The RN will conduct glucose tolerance tests, ensuring patient safety and accurate results. They will offer culturally sensitive nutritional counseling, including dietary approaches such as the Mediterranean diet. The RN will provide patient-specific exercise counseling tailored to individual needs and limitations. They will instruct patients on the implementation, usage, and troubleshooting of continuous glucose monitors, integrating devices with mobile technology and facilitating data sharing for remote consultations. The RN will upload data from patient home monitoring devices so the data is available to the clinical team during in-person and virtual visits. They will teach insulin administration, management of high and low blood sugar, sick day rules, and glucagon pen usage. The RN will provide in-depth training on different continuous glucose monitor brands to ensure patient proficiency. They will troubleshoot issues related to insulin pumps and coordinate with distributors for pump authorizations while providing technical and emotional support. The RN will assure patients have adequate supplies on an ongoing basis to avoid interruptions in care. They will educate patients on insulin pump usage features and maintenance. The RN will respond to medical advice inquiries regarding diabetes management. They will collaborate with the diabetes care team to identify diabetes distress and ensure timely referrals. The RN will work alongside the registered dietitian and health coach to ensure a comprehensive approach to patient care. They will uphold and promote excellence in diabetes care in partnership with endocrinologists and nurse practitioners. The RN will work Monday, Wednesday, and Friday from 8:30 AM to 5 PM, with sessions on Tuesday and Thursday ending at 7 PM. The RN will cover one to two Saturdays a month from 8 AM to 4:30 PM. Proficiency in electronic health records and medical software is required. Strong interpersonal and communication skills with an emphasis on patient-centered care are essential.
