BID DUE DATE: 7/17/24 5pm EST Facility Location: Sierra Nevada Memorial Hospital AUGUST 11-17 SEPTEMBER 7th, 8th, and 10th-14TH NEED COVERAGE especially 15th and 16th 22nd, 24th-28th OCTOBER 29th and 1st-5th 6th, 8th-12th NOVEMBER 3rd, 5th-9th 10th, 12th-16th 17th, 19th-23rd 24th, 26th-30th DECEMBER 15th, 17th-21st 22nd, 24th-28th Setting: General Acute Care Hospital (Stroke Certified, not Chest Pain Center accredited, not a Trauma Level designation) Duties: Hospital Coverage, we have a need for a Radiologist to cover the Hospital which involves the IR needs along with Stroke CT's and other procedures/availability. Required Procedures: IR Procedures (Thoracentesis, Paracentesis, Nephrostomy Tubes, Intra-abdominal abscess drainage, Chest Tubes, Suprapubic Catheters); CT/US Biopsies (Thyroid, Lung, Other), Barium studies, Lumbar punctures. EMR: Cerner Support Staff: Diagnostic Imaging (Radiology) Nurses and Techs; We do have a large Radiology group in Sacramento that performs overflow and after-hours reads Reason for Coverage: Resignation of Radiologist Dealbreakers: Procedural Skills, Acute Care Imaging capability (Stroke CT's, ED and ICU imaging studies) Travel Preferences (see attachment for specific travel guidelines): Preferred Lodging: No preferred hotels. In the event that the CommonSpirit Health preferred hotel is not available, hotel/lodging rates will be reimbursed based on actual expense, up to 1.5 times the current GSA rate for the market. Additional Details / Questions Answered by Facility: Admissions per day: 12-15 Patients/day: ED Daily Volume is 85-90 patients/day, Average Daily Inpatient/Obs Census is ~50 patients (includes ICU and OB). Types of cases: Surgical, Medical, ICU, and OB TOTAL: ~50 studies/day + 5-8 procedures/day BREAKDOWN of Daily Work CT's: 15-20 XRays: 15-20 US's: 10 MRI's: 5 IR: 5-8 procedures ####s to Submit (see attachment for specific rate structure guidelines): Malpractice - Must be included in rate. Orientation - half the hourly rate, kicks in after 8 hours. - "Per hour of orientation or training beyond 8 hours" Hourly Holiday rates - 1.5x the regular rate per CS guidelines for all corresponding rates. Night call (2.5 hours gratis) Call back - Must match hourly rate - "per hour of patient contact hours after gratis hours are completed, gratis prorate to MSA terms" Tier 1 (will be reviewed by client first): Hourly/callback under ####50; Night call under ####00 Tier 2 (will be reviewed by client after Tier 1): Hourly/callback ####51-625; Night call ####00-1400 Schedule/Availability Requirements: Schedule/Shift times: 8a-430pm with potential night call after 430p 14 shifts/month (4 of those days will be the weekend) Any call? To process the best presentations, Account Coordinators and Account Managers will kickback presentations that do not include the information below (if it has not been discussed already). If you are unable to include something in the presentation, detail why and the plan to get it sent over. Account Managers and Account Coordinators will take this into account as they review the presentation, though they still reserve the right to redirect it back to the vendor if they deem it necessary. Copy of state license and the status. If provider's license in any other status besides active speak to Account Manager before presenting. Certification (select multiple that apply; this pulls to the cover page for the Client) Availability Section - what is the providers ongoing availability (this pulls to the cover page for Client) Answer the pertinent questions: Has the provider's professional license or certification been investigated or suspended? (pulls to the cover page for the Client) Has the provider been convicted of, or charged with, a crime other than a minor traffic violation?
