When you thrive, so does the community of care we're building together.  Offer Based on Years of Experience What You Need to Know: Be the Best Palliative Nurse Practitioner Lead You Can BeIf you meet these qualifications, we want to meet you!Graduated from an approved school of professional nursingOne year experience as a Nurse Practitioner required in CAMaster's degree, requiredPrevious experience in hospice, home health, general medical surgical or patient population specific (i.e.: geriatrics, pediatric, etc.) nursingMinimum three years' Palliative Care services experienceDemonstrates familiarity with policies of the agency and state(s) and federal regulations pertaining to nursing services.Demonstrated expertise in pain/symptom management and advanced care planningRequired Certifications and LicensureLicensed to practice as a Nurse Practitioner in the state of agency operation with current and unencumbered advanced practice nursing license in state(s) practicingActive DEA license in the state where care is being provided.Certified by a nationally recognized advanced nursing practice organizationPossess and maintain valid CPR certification while employed in a clinical role, preferredMust be a licensed driver who can travel to all business locationsMust have reliable automobile, valid driver's license, and the minimum state required liability auto insuranceResponsibilities:  As a Palliative Care Nurse Practitioner Lead, you will: Develop and implement clinical plans of care for patients with chronic and terminal illnessesPerform patient assessment, history and physical; recommendations on aggressive symptom-managementMonitor patient progress; update the Medical Director, Team Director and team members; consult regarding discharge plannning; and patient/family teachingMaintain communication with the supervisory physician (s) and attending physician to review patient records and care plans on a routine basisInitiate communication with attending physicians, other Palliative Care staff members and other agencies as needed to coordinate optimal care and use of resources for patient/familyMay provide support at time of deathPrescribe medication as permitted by the state and/or the State Board of NursingParticipate in continum of care for patients that are hospice appropriate and ongoing education programs.Leadership ResponsibilitiesSupervise and delegate to members of the care team as appropriateRun monthly meetings with providersOn a monthly basis measure provider productivity and referral flow from provider organizationsJoin the monthly Palliative Care calls with national and site leadership, where productivity, conversion rate and any other hot topics will be reviewedParticipate in the interview process for any new Palliative Care positionsCollabore with PC orientation team for onboarding, orientation and education of all new staffCollaborate with the Director of Palliative Care on hospital based and operational outcomesCo-lead the daily Palliative Care standup callOur Investment in You  Caring for others starts with caring for you.
