Registered Nurse Patient Navigator Opportunity Full Time / $10k Sign on Bonus / Housing Assistance Available MLR is looking for a dedicated and skilled Registered Nurse Patient Navigator to join our client's hospital team near Grant, Colorado!RN Patient Navigator - Key Responsibilities: Facilitate care coordination, supporting patients throughout their healthcare experience, from diagnosis to recovery and follow-up. Work hours: 8:30 AM - 5:00 PM. Inform patients and their families about medical treatments, procedures, and available support services. Collaborate with healthcare teams to create and implement tailored, patient-centered care plans. Ensure thorough and accurate documentation of patient care, keeping records up-to-date for healthcare providers. Qualifications for an RN Patient Navigator: Active RN license in Colorado, or eligibility to secure one before starting. Associate Degree in Nursing (ADN) is mandatory. Basic Life Support (BLS) certification (can be obtained upon hire). Advanced Cardiac Life Support (ACLS) certification is required. A minimum of 3 years of nursing experience, especially in inpatient or outpatient orthopedic settings.
