About the Position: We are looking for skilled and compassionate RN Case Managers to join our dynamic team. This role offers a starting pay rate of $55.91 per hour and comprehensive benefits through the 1199 Union, including health, dental, vision, and retirement plans. Key Responsibilities: Conduct comprehensive assessments of patients' needs, including medical, psychological, and social factors. Develop and implement individualized care plans in collaboration with patients, families, and interdisciplinary teams. Monitor and evaluate patients' progress and make necessary adjustments to care plans. Facilitate effective communication between patients, families, and healthcare providers. Advocate for patients' needs and ensure they receive appropriate services and resources. Document and maintain accurate records of care activities and outcomes.
