Job Description & Requirements Specialty: Case Management Discipline: RN Start Date: 04/06/2026 Duration: 11 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Unit Home Health Case Manager RN Unit Type Home Health Care Setting Post Acute Cancellation Policy 2 weeks notice Guaranteed Hours 32 hours per week Required Certifications BLS Certification Years of Experience 2 Years Required Certifications BLS Certification Shift Notes Monday through Friday 8a-430p; Every 4th week on call, call is Monday-Friday 430p-8am and then Friday 430p-Monday morning at 8am.; Every 4th holiday Charting System Point Click Care (PCC) No on call or call back available Guarantee (Clinical): 36 hours per week for twelve-hour shifts, or 40 hours per week for eight to ten hour shifts.
