Nurse Practitioner (Adult/Geriatric) Location: Dutchess country, NY ( specifically Poughkeepsie and Wappingers Falls) Job type: Full-Time or Part-Time | Day Shift Only Shift: Monday - Friday (No nights or weekends) Job Summary: We are seeking a dedicated Occupational Therapist specializing in upper extremity rehabilitation to help patients restore functional skills, reduce pain, and improve quality of life. The role involves developing individualized treatment plans and delivering patient-centered care in a collaborative clinical environment. Job Summary & Responsibilities: Perform comprehensive health assessments including physical exams, medication reconciliation, and medical history reviews Develop and manage individualized care plans in coordination with the interdisciplinary team Diagnose and manage chronic and acute conditions with appropriate interventions and referrals Order and interpret diagnostic tests (labs, imaging, etc.) Prescribe medications and treatments in accordance with state and facility guidelines Participate in the initiation of chronic care management services Educate residents and their families on disease prevention, treatment plans, and self-care strategies Collaborate in quality improvement initiatives to enhance care delivery and outcomes Maintain accurate and timely documentation in the electronic health record system Education, Licensure, and Experience: Master's Degree in Nursing from an accredited Nurse Practitioner program Active and unrestricted Nurse Practitioner license in the State of New York Current DEA license for prescribing medications Eligible for Medicare and Medicaid billing Preferred: Minimum of 3 years of clinical experience, particularly in geriatrics or long-term care Preferred: Familiarity with eClinicalWorks (ECW) or similar EHR systems Benefits: All standard benefits For more information please reach out to Valeria Moore: at 469-331-2893 or by Email: at
