Job Title: Health Information Management SpecialistDepartment: Health and Human Services (HHS) Reports To: Business Office ManagerLocation: Health and Human Services BuildingLevel/Status: Non-Exempt/Full-Time SUMMARYThe Health Information Management Specialist is responsible for the accurate management, security, and organization of medical records to ensure HIPAA compliance, support care coordination, and promote efficient access to health center data and patient information. This position assembles, scans, maintains, updates, retrieves, and copies confidential health records, and processes requests for the release of medical information to internal and external entities in a timely and compliant manner. The Health Information Management Specialist works collaboratively with administrative and clinical staff to maintain high-quality medical and behavioral health documentation. Responsibilities also include supporting system improvements, participating in staff training related to documentation standards and processes, and assisting with data collection efforts to enhance organizational performance. ESSENTIAL QUALIFICATIONSAssociate Degree or Bachelors Degree in Health Information Technology, Health Administration, or related fieldCertification as a Registered Health Information Technician (RHIT) or Certified Medical Records TechnicianThree (3) years of Health Information Management professional experienceExperience working with Electronic Health Records (EHR) systemsMust have thorough knowledge of confidentiality of patient records: medical, psychotherapy and substance abuse, HIPAA, and confidentiality of substance abuse records within 42 CFR part 2DUTIES AND RESPONSIBILITIES include, but are not limited to the following:Reviews records for completeness, accuracy, and compliance with regulationsImplement and maintain policies for data retention and destruction.Act as privacy officer for department, develop and enforce privacy and security protocols, manage access controls to health information systems, and investigate and report data breaches when they occur.Work with HHS providers to ensure all requested health information is sent correctly and timelyCompile and analyze health data for quality improvement initiatives in collaboration with the Clinical Coordinator and Assistant HHS Director including performance metrics, patient outcomes, and population health management.Respond to requests for copies of protected health information in accordance with HIPAA privacy regulations; this includes the ability to analyze authorizations for HIPAA compliance and a working knowledge of what medium to release records onUnderstands and follows all HIPAA and privacy guidelines for the management and Release of Information (ROI) for electronic or physical medical records Enters data, such as patient demographics, into appropriate systemsSort and file or scan loose paperwork in patient charts maintaining proper order; ensuring records are updated and complete with required forms and documents Prepare chart conversion; consolidate, repair or establish new charts as needed.Abstract information from the medical record for chart review, monitor deficiencies and notify supervisor as appropriate Effectively uses the Electronic Health Record (EHR) to:Gather data for release, reviews all releases for accuracy and thoroughness to ensure appropriate information is releasedComprehensively tracks all ROI's receivedEffectively communicates with all parties to ensure a smooth process for releasing informationCompletes all ROI in a timely and efficient mannerClearly tracks and documents all information that has been released and date of releaseAs a designee of the Business Office/CHB Specialist , the HIM Clerk completes requests for information by sending HHS patient records to the intended recipientsProcesses outside requests for labs, disability, the transfer of medical and/or mental/behaviroal health records, and pharmacy requestsResponsible for staff training in confidentiality and HIPAA, documentation standards, and compliance.Plan for technology upgrades, EHR migrations, and interoperability.OTHER SKILLS AND ABILITIESWorking knowledge of medical terminologyStrong attention to detailGood customer service skillsProficient in data entry and computer skillsICD-10-CM knowledgeableThis occupation involves using communication and organizational skills to coordinate or train others to accomplish goalsExperience with EPIC EHR strongly preferredCONDITIONS OF EMPLOYMENTCPR Certification, can obtain after hire - within 6 months.
