This role is designed to support individuals transitioning from institutional care or those newly referred for services, by focusing on health-related tasks, personal care assistance, and ensuring clients have access to the resources they need. This position provides the flexibility to work in home-based environments, offering direct support to clients in need while adhering to the standards set by 4th Second. This position provides rapid response services when required, interacts with the 4th Second medical team and helps to implement the client's Plans of Care, providing feedback and input to both the client and 4th Second staff on client progress, issues, barriers, and other relevant areas.Supervisory Responsibility: NoneJob Type: Full-time, Hourly, Non-exemptExpected schedule: Hours of operation 8 am - 6 pm; 35 per week (7 hour workday); weekends and evenings required (e.g., Sunday-Thursday, Tuesday-Saturday)Location: Vallejo, CA - residential households & communityBenefits:403(b) Retirement planEmployer Sponsored Kaiser Platinum Health Insurance, Dental, Vision, and Life Insurance20 Days Paid time off per year12 Paid HolidaysEmployee Assistance Program1 Hour Employer Paid Wellness TimePet insurance discountsWhat You Will Do:Assist with personal hygiene tasks including bathing, grooming, dressing, and toileting to support client dignity and daily functioning (if required)Support mobility and transfers such as assisting clients in and out of bed or chairs and providing standby support during ambulationPerform light housekeeping duties such as sweeping, mopping, laundry, and cleaning to maintain a safe and sanitary living environmentPrepare and serve meals based on client preferences and dietary restrictions, including cleaning up after mealsEscort or accompany clients to appointments or errands using public transportation or walking, ensuring safety and comfort throughoutMonitor and document basic vital signs including blood pressure, temperature, and, if applicable, blood glucose levels, according to care plan requirementsRemind clients to take prescribed medications as directed in the care plan without administering or managing the medicationObserve and report changes in client condition including physical, behavioral, or environmental shifts to the assigned supervisor or care teamMaintain accurate records and documentation for each visit, including task completion, vitals, and any concerns notedBuild trust and rapport with clients by using empathetic, culturally competent, and trauma-informed approaches that promote client comfort and cooperationMinimum Qualifications: High school diploma or equivalentCertification as a Home Health Aide (HHA), Nursing Assistant (CNA), or completion of a Personal Care Aide (PCA) training programMust pass a background check and tuberculosis (TB) testMust be available to work full-time, including some weekends and agency holidays, based on client needsHave an in-depth knowledge of in home support services responsibilities, standards, and proper techniques, and proven ability to demonstrate proper techniquesPossess strong interpersonal communication skills with ability to communicate effectively/professionally with medical personnelAccess to reliable personal transportation required, including a DMV record that permits the driver to be insured under 4th Second's automobile coveragePass Required Background Checks: Successfully complete Live Scan and other background checks as a prerequisite for hiring, ensuring compliance with safety and security protocols set forth by 4th SecondPreferred Qualifications: Experience working with target populations such as individuals from diverse cultural, socioeconomic, and linguistic backgrounds, particularly those facing chronic illness, disabilities, or mental health conditionsFamiliarity with Motivational Care Management models and how to apply them within a care planWillingness and ability to work with individuals facing chronic illness, active substance addiction, low self-compliance with health plans, and other complex social or health challengesExperience working with Medi-Cal beneficiaries and knowledge of CalAIM Community SupportsPhysical Qualifications: Able to be physically active for duration of work shift, excluding breaksAble to lift 40 lbs.Able to push or pull individuals in wheelchairAble to carry 25 lbs. up one flight of stairsAble to walk up and down a minimum of one flight of stairsAble to bend downward and forward at the leg, waist, and spineAble to kneel on one or two legsAble to stand for extended periods of timeAble to extend hands and arms in any directionWork Environment:This position operates in both residential care settings and the community. The role may involve exposure to unsanitary conditions, challenging behaviors, and various environmental factors.
