Job Title: RN Case Manager-{167889} Location: Saddle Brook, NJ 07663 Coverage Area: Bergen County, NJ Benchmark: 30 cases per week (Monday-Friday) Mileage Reimbursement: $0.43/mile Pay Rate: $56.14/hour Schedule: Mon-Fri, 8am-5pm | Weekend On-Call Overview We are seeking a dedicated RN Case Manager to support high-risk patient populations in Bergen County, NJ. This role focuses on delivering comprehensive, patient-centered care through coordination, assessment, and collaboration with interdisciplinary teams. The RN Case Manager will play a key role in improving patient outcomes, promoting cost-effective care, and ensuring smooth care transitions.
