Job Title: RN Case Manager-{167889} Location: Saddle Brook, NJ 07663 Coverage Area: Bergen County, NJ Benchmark: 30 cases per week (Monday-Friday) Mileage Reimbursement: $0.43/mile Pay Rate: $56.14/hour Schedule: Mon-Fri, 8am-5pm | Weekend On-Call Overview We are seeking a dedicated RN Case Manager to support high-risk patient populations in Bergen County, NJ. This role focuses on delivering comprehensive, patient-centered care through coordination, assessment, and collaboration with interdisciplinary teams. The RN Case Manager will play a key role in improving patient outcomes, promoting cost-effective care, and ensuring smooth care transitions. Key Responsibilities Perform comprehensive assessments of high-risk patients Develop and implement individualized care plans in collaboration with primary care providers & continuously evaluate appropriate, cost-effective levels of care Coordinate care transitions with physicians, social workers, discharge planners, and other healthcare professionals Arrange non-medical support services (e.g., housing, transportation) to support treatment compliance Maintain accurate documentation of clinical, functional, and financial outcomes Identify opportunities for health promotion and illness prevention Proactively prevent adverse events and intervene when necessary Participate in weekly case conference meetings (Wednesdays at 8:45 AM) Required Skills & Qualifications Active, unencumbered Registered Nurse (RN) license Associate’s or Bachelor’s degree in Nursing or related field Strong clinical assessment and critical thinking skills Proficiency with technology and Microsoft Teams HIGHLY Preferred Experience Case Management certification Experience with Home Care Home Base (HCHB), PointCare, or PointClickCare Prior experience in home health or case management settings Job ID: 167889 Keywords: RN Case Manager, Registered Nurse, Care Coordination, Case Management, Home Health, HCHB, PointClickCare, PointCare, Patient Assessment, Care Transitions, Bergen County NJ, Microsoft Teams, Clinical Documentation, High Risk Patients
