Primary Care Nurse Practitioner - Hybrid Remote Looking for an NP opportunity that offers flexibility, innovation, and the chance to make a real impact in chronic disease management? We have a few full time, permanent openings in the North-Central Utah region. The Role This is a primarily telehealth-based position; most of your work will be done virtually, with the occasional option to see patients in person if they're local to you. You'll be part of building something new: an innovative virtual and in-person care model focused on providing primary care for patients with (CKD) and related chronic conditions. Primary Conditions you'll manage: Chronic Kidney Disease (CKD) Hypertension Diabetes Heart Failure Schedule & Lifestyle Monday-Friday only Self-managed schedule No nights, no weekends, no on-call Requirements: Minimum 5 years of NP experience (primary care, internal medicine, cardiology, nephrology, or case management) Experience managing chronic diseases (CKD, CHF, diabetes, hypertension) DEA license (or ability to obtain within 90 days) Current CPR/BLS certification Comfortable working across multiple EMR systems Strong computer skills - MS Word, Excel, Outlook Preferred Qualifications Telehealth experience (strong plus) Value-Based Care or Population Health background Medicare Advantage risk documentation/coding experience Experience reducing avoidable hospitalizations through proactive, coordinated care Familiarity with palliative care discussions If this sounds like the right fit, let's connect!
