Headquartered in Chicago, IL, we currently operate in over 10 states and are rapidly expanding throughout the USA.   Job Overview: Vitae Health Systems is seeking a highly organized and detail-oriented VBC Operations Coordinator/Analyst to support the operations of our Value-Based Care program. The ideal candidate will have a working knowledge of healthcare data and operations, and is comfortable learning new program mechanics quickly. This role is critical to keeping our program running day to day, and the right person will take pride in the details that make that possible. This is a hybrid role based in the Chicagoland area, reporting directly to the Director of Value-Based Care Operations.   Key Responsibilities: · Maintain program dashboards, performance trackers, and reporting tools that give leadership clear visibility into program activity and outcomes · Monitor and organize data inputs from multiple sources including claims, census feeds, and utilization reports; flag discrepancies and ensure accuracy · Communicate care gap alerts and documentation flags to nurse practitioners and clinical staff based on defined protocols; track follow-up and escalate unresolved items to clinical and operational leadership · Prepare recurring program submissions and supporting documentation including provider rosters and beneficiary alignment materials · Track provider participation and engagement against program benchmarks; produce regular status reports for leadership review · Support beneficiary alignment campaigns including logistics coordination and field travel · Support program compliance by maintaining accurate records, following defined documentation standards, and flagging issues through appropriate channels · Additional responsibilities may be assigned as the program evolves   Required Qualifications: · Bachelor's degree in Healthcare Administration, Business Administration, or a related field; or a minimum of 4 years of experience in healthcare operations or a related role within a skilled nursing, long-term care, or post-acute care setting · Proficiency in Microsoft Excel and comfort working with healthcare data · Experience working in a setting with regular interaction with clinical providers such as nurse practitioners, physicians, or therapists · Strong attention to detail, organizational skills, and ability to manage multiple priorities simultaneously · Clear written and verbal communication skills; able to adjust style and detail level for clinical and operational audiences · Ability to travel as needed, approximately 1 to 2 weeks per quarter   Preferred Qualifications: · Familiarity with CMS value-based care models or CMMI innovation programs · Experience working directly with nurse practitioners or advanced practice providers · Data visualization experience using Power BI, Tableau, or equivalent tools · Exposure to risk adjustment, chronic condition coding, or claims-based performance reporting   Why Join Vitae Health Systems? · Opportunity to play a meaningful role in advancing value-based care for a high-need Medicare population · Be part of a rapidly growing organization with a commitment to excellence in healthcare · High-visibility role with direct access to senior leadership and real opportunity to grow with the program · Competitive salary and benefits package · Collaborative and supportive work environment
