This contract role is perfect for someone who thrives on independence, embraces clinical challenges, and LOVES helping patients in the comfort of their own homes.If you ve got rock-solid HCHB (HomeCare HomeBase) skills and can manage a full caseload like a pro this is YOUR opportunity. Schedule + Contract Details:Start: 5/4 or as soon as credentialedShift: 7 ON 7 OFF_ Monday thru Thursday 5pm to 8am and then Friday 5pm to Monday 8am,_ there is a second nurse on call with them on the weekends.Territory: Oshkosh WI areaContract: 13 Weeks to Start High Chance of ExtensionCompensation: Up to $102,900 per year PLUS mileage reimbursement & full benefits Expectations: 20 to 25 visits per week and 40HRS/ per week guaranteedResponsibilities:Complete initial & comprehensive assessments to determine patient eligibilityCreate and implement individualized care plans with the patient s providerUse sound clinical judgment and document accurately via HCHBCommunicate regularly with the care team, scheduling, and leadershipBe the calm in the chaos delivering compassionate, professional care while keeping the workflow moving efficientlyRequirements Qualifications:2+ years working in Home Health Hospice setting visiting multiple patients per day!HCHB (HomeCare HomeBase) Highly PreferredActive RN license in Wisconsin or Nursing Compact LicenseNOTE: NLC or State License MUST be active at time of application, submissionWhat We Offer:Competitive weekly pay with guaranteed hoursWeekly direct depositPremium healthcare benefits24/7 recruiter support yes, really!Housing/travel assistance if neededContract extension opportunities build your career with us!This is your moment.
