Responsibilities:As the MDS Coordinator/Nurse Leader at Gunnison Valley Health Senior Care Center, you will assume responsibility for the accurate and timely processing of MDS forms. You'll utilize your unique skills and ideas by:Ensuring complete and accurate completion of the RAI process (including assessment and care plans)Maintaining resident records while adhering to State and Federal regulationsAssisting in the admission, transfer and discharge of residents as necessaryPerforming administrative duties such as completing medical forms, reports, evaluations, studies, charting etc. as necessary.Working in collaboration with other staff members and departments Call Requirements every third weekend and rotating weekends. 50/50 MDS processing and Assistant Director of Nursing dutiesRequirements:To be seriously considered for this role, you'll need to have:Associate degree in Nursing from an approved professional nursing school required.1 year experience in a hospital, skilled nursing facility, or other comparable institution. Must have an understanding of the MDS process. Prefer approved MDS course completion certification.
