Community Connections, Inc. has an exciting position with our new community care team, Southwest Colorado Cares (SWCC). The SWCC team seeks a part-time (20 hours a week) Care Navigator to help community members access the resources they need and want. This is a hybrid position, part-time, with flexibility in schedule. It is hourly and non-exempt.  Hourly wage is $25.49, plus travel reimbursement. Part-Time benefits include: 401K participation after 12 months of employment and 1,000 hours worked within those 12 months; Employee Assistance Program (EAP); unlimited Personal Time Off (PTO). EOETitle: SWWC Care NavigatorReports To: Senior Director, Southwest Colorado Cares FLSA:  Hourly, Part-Time, non-exempt: 20 hours per week Location:  Hybrid position for residents of Archuleta, Dolores, La Plata, Montezuma, or San Juan Counties Hourly Rate: $25.49, plus travel reimbursement Job Description: The part-time Care Navigator will receive referrals from community partners, health care providers, or the Southwest Colorado Cares website to help referred clients identify needed services, navigate services and develop goals. The Care Navigator will work with clients to address basic referrals and coordination as well as develop goals and plans for more complex member needs. This position will be familiar with health and community resources that will reduce barriers to care and will deliver culturally appropriate information to address social drivers of health outcomes. This position is a part of the Southwest Colorado Cares Team (SWCC) within Community Connections, Inc. SWCC is a new team working to improve the quality of resource referrals and care coordination in Southwest Colorado.    Key Responsibilities: Serve as community liaison and maintain relationships with key individuals in the community and serve as an advocate by coordinating linkages or referrals to improve health, social, and environmental conditions for members.   Connect members with appropriate resources and manage the successful completion of referrals, assisting with warm hand-offs and documenting in Channels360 system. Facilitate access to preventive and disease management health services.   Participate in interdisciplinary care team meetings as indicated.    Manage difficult to reach and non-compliant members.   Communicate the mission and role of the SWCC team and Care Navigation to community associations, community-based organizations, senior groups, local clubs and groups, and churches.    Work collaboratively with and seek guidance from the Senior Director and/or Vice President of Case Management as needed to develop a plan of management associated with health care goals for each member addressing the diverse needs in a culturally appropriate way.   Maintain confidentiality and use only the minimum amount of protected health information (PHI) necessary to accomplish job related responsibilities. Maintain confidentiality of patient information.     Participate in staff meetings, case conferences and in-services. Maintain familiarity with all policies and procedures that impact decisions and care.   Support documentation of member or partner organizational information in internal systems (Channels360 and CRM) as needed. Support regional resource directory as needed.  Perform additional duties as assigned by the Senior Director.   Required Qualifications:   Four (4) years of relevant experience in the case management or care coordination; OR A bachelor’s degree in social work, social sciences, human services, counseling or related field; ORThe combination of education and experience that would enable performance of the full scope of the position;2 years of progressive related experience working with diverse populations, community or faith-based organizations  Health care setting experience preferred  Bilingual in English/Spanish preferred  Core Competencies: Compassion and empathy Listening Motivating others and working well in a team setting Creative problem solving Persistence and patience Time management Knowledge and Abilities:   Knowledge of health education, motivational strategies, and an empathetic manner working with the underserved.  Knowledge of healthcare business or system preferred  Knowledge of social and health resources in the five southwest Colorado counties (Archuleta, Dolores, La Plata, Montezuma, San Juan) Computer literacy and intermediate knowledge in Microsoft Excel, Word, & Outlook Ability to use standard office equipment Ability to manage assigned/referred caseloads, set priorities, and manage time effectively Capability to set goals with clients, manage relationships with community partners and care teams, and persevere with hard-to-reach clients Capability to problem solve, learn quickly and work with minimal oversight Ability to handle and maintain confidential information Excellent communication skills, including written, phone, and customer service skills Ability to commute for business, when needed. The employee will be required to attend meetings, training and other community activities within the region of Southwest Colorado, with the possibility of occasional travel within Colorado or out of state to training or conferences.
