Northern Manhattan Rehab and Nursing Center - Now Hiring MDS Coordinator (RN) Northern Manhattan Rehabilitation and Nursing Center is seeking a detail-oriented and compassionate MDS Registered Nurse (RN) to join our interdisciplinary team. In this role, you will be responsible for coordinating and completing the Minimum Data Set (MDS) assessments to ensure accurate care planning, regulatory compliance, and optimal reimbursement, while supporting high-quality, resident-centered care. Comprehensive benefits package to include health, dental, and vision insurance Generous paid time off (PTO) Supportive and collaborative work environment Opportunities for professional growth and advancement A team dedicated to delivering exceptional care to our residents Responsibilities of MDS Coordinator (RN): Coordinate and complete MDS assessments in accordance with federal and state regulations Develop and update comprehensive care plans in collaboration with the interdisciplinary team Ensure accuracy and timeliness of all MDS documentation and submissions Monitor clinical documentation to support appropriate case mix and reimbursement Participate in care plan meetings and communicate resident needs effectively Collaborate with nursing, therapy, and administrative staff to ensure regulatory compliance Requirements for MDS Coordinator (RN): Current Registered Nurse (RN) license in the state of New York Prior experience with MDS assessments in a skilled nursing facility preferred Strong knowledge of RAI process, care planning, and Medicare/Medicaid guidelines Excellent organizational, analytical, and documentation skills Strong communication and teamwork abilities Commitment to providing high-quality, resident-centered care Northern Manhattan Rehabilitation & Nursing Center is a diverse and dynamic 320-bed long-term care facility committed to service excellence and outstanding resident and patient outcomes.
