House Call Nurse Practitioner Brooklyn, NY Full-time Deliver personalized home-based primary care from a Brooklyn hub, focusing on homebound patients throughout NYC. Position Overview A large multispecialty group in NYC is seeking a Nurse Practitioner (NP) to provide high-quality, patient-centered care in patients' homes and nursing facilities. This role is ideal for providers who enjoy holistic care, chronic disease management, and direct patient engagement .
