Rapids & Grandville MI In this position you will play a key role in overseeing infusion care provided by nurses in our ambulatory infusion centers in Grand Rapids & Grandville MI and via telehealth as needed You will create and maintain individualized care plans for all patients referred to the center ensuring safe and effective care during infusion procedures You will also serve as the primary clinical liaison between the referring physicians office and the infusion center providing critical support to the team and evaluating patients needs throughout the treatment process Your expertise DR SW Grandville MI 49418Primary responsibilitiesReviews and evaluates clinical information provided by referring physician offices and admits appropriate patients for infusionEvaluates newly referred patients and develops initial plans of careMonitors plans of care and makes necessary changesPerforms pertinent physical exams with assessment of normal and abnormal findings on new patients and confirms suitability for infusionReviews all lab reports provided by the referring physicianRequests diagnostic and lab studies of the referring physician upon evaluation of the patient as neededRecognizes situations which require the immediate attention of a physician and initiates life saving procedures when necessaryConsults with the supervising Medical Director as is necessaryDocuments infusion orders in the electronic medical recordDictates or documents notes in the electronic medical recordSecondary responsibilities Promotes company quality outcome initiativesDelegates responsibilities to staff nurses on dutyFollows company policies and CDC guidelines for infection controlCarries out all duties in a professional and courteous mannerProvides patient education and answers questions from prospective patients and referring physiciansEnforces all HIPAA compliance guidelinesQualifications Education and Experience Current and valid state professional nurse licensure requiredGraduate of accredited Nursing AssociatesBachelors degree requiredCurrent CPRBLS certification requiredMinimum of 1yr IV experience requiredInfusion experience preferredCertification in specialtyCRNI Certified RN Infusion preferredICUED experience preferredSkills Must have a current license to practice as a Nurse Practitioner or Physicians Assistant in the state in which service is providedMust have current DEA licenseInfusion rheumatology oncology or neurology experience preferredMinimum of 2 years working in a clinical setting requiredMust possess outstanding communication and interpersonal skillsStrong customer service skills and a commitment to prioritizing the comfort and needs of patientsIntermediate computer skills MS Office and EMR experience preferably additional experience working with telehealth systemsStrong time management skills and ability to work independentlyDemonstrated ability to work collaboratively a high degree of
