Family Stabilization & Support services ensure that all the family's goals are met over the contract period in safe and nurturing home environments. CSR provides immediate concrete services, which include support in parenting, medical, mental health and community access supports; educational/vocational career assistance, as well as supports with legal issues: domestic violence, probation obligations, court ordered CRA issues and follow-through. Family home visits will be conducted weekly (1-2 times) under contract referral. Families are provided immediate assessments, advocacy and collaborative updates by CSR Staff with DCF collaterals.Essential Duties and Responsibilities include the following: In person home visits with the referred family to help achieve goals outlines in DCF service requestCredible System: Written narratives of home visits weekly over the period of contract, written documentation of calls/contacts by telephone (daily/weekly)Produce Monthly Report, every 30 daysVirtual Gateway System: Completion and implementation of Treatment Plans (each 3-month period) w/ appropriate activities against the goals identified by DCF, complete Progress Review (each 3 month period)Maintain neat and orderly filesMajor Measurable Goals:Weekly home visits executed flexible for the family, at times after 5pm (meeting and connecting with each parent where they are.)Weekly collaboration with collaterals (DCF, Service providers, etc.)Response to emergencies (Child Crises, Hospitalizations, Emergency Meetings, School Meetings, etc.) when necessaryDCF Case Conference participation to review progress of goalsWeekly supervision with Program DirectorProfessional representation alwaysAppropriate attitude and team spirit in the office and in the communityAppropriate boundaries in relationships external to CSR (Parents, children, DCF providers)Work ethic (maintain work hours expected, home visits, appropriate lunch hours)Position Challenges:Time Management (Flexing)Good Operating Vehicle (Car)TeamworkStrong skills in communication (verbal/written)Timely paperworkNecessary availability after 5pm and before 8amConfidentiality and Mandated Reporter statusQualifications:Bachelor's degree in Social Work or related Human Services field required.
