Participates in Quality Improvement (QI) projects in collaboration and coordination with the HIV team. Assists in coordinating team-based QI projects. Any and all other duties as assigned.Qualifications and Education Requirements:Physician, Nurse Practitioner, or Physician Assistant as their primary job description.Prior training in primary care (MD/DO residency, NP residency, or clinical experience).Familiarity with clinical practice in Patient-Centered Medical Home model of care required. Note: Fellow will be 1.0 FTE employed at FHCW with 0.4 FTE clinical. Reporting to the Provider Leader -Special PopulationEOEMonday-Friday; 8:30am -5:00pm
