Job Description

The Mental Health Therapist provides outpatient mental health services to children, adolescents, and adults in a community health center setting. Services include assessment, diagnosis, and treatment of emotional, behavioral, and relational concerns, delivered in accordance with Medicaid and payer requirements, clinical competency, and organizational policy. The therapist works as part of an integrated, multidisciplinary care team to support whole-person care across the lifespan. Duties include: * Provide individual, family, and group therapy to children (starting at age 5), adolescents, and adults, utilizing evidence-based, trauma-informed, and developmentally appropriate therapeutic interventions. * Conduct biopsychosocial assessments, diagnostic evaluations, and ongoing clinical reassessments to support medical necessity and treatment planning. * Develop, implement, and regularly update individualized treatment plans in collaboration with patients and, when appropriate, caregivers or family members. * Address a broad range of mental health conditions, including anxiety, depression, trauma-related disorders, behavioral concerns, adjustment disorders, and family or relational issues. * Provide crisis intervention, safety planning, and referrals to higher levels of care as clinically indicated. * Maintain competency in clinical practice by engaging in ongoing professional development, continuing education, and review of current research and best practices in child, adolescent, and adult counseling. * Collaborate with primary care providers, care coordinators, case managers, and community partners as part of an integrated care model. Participate in the organization's referral process by accepting patients or making referrals for patients who may need other services (e.g., medical, dental, vision, psychiatric). * Maintain timely, accurate, and compliant clinical documentation in the electronic health record (EHR). Enters case history and progress notes within the three-day encounter sign-on period. Modifies treatment activities or approaches as needed to comply with changes in clients' status. * Maintain a full patient schedule (i.e., 1 patient per hour). * Participate in case consultations, interdisciplinary meetings, and quality improvement activities. * Educate patients and families on mental health conditions, treatment options, coping strategies, and available community resources. * Adhere to all ethical standards, professional guidelines, and applicable federal and state regulations.
