Job Description

Family First Health is seeking a Full-Time Licensed Mental Health Professional to join our Lebanon office as a Behavioral Health Consultant within our integrated primary care team. The Behavioral Health Consultant (BHC) works side-by-side with primary care providers and care team members to deliver brief, evidence-based behavioral health services in a fast-paced medical setting. Using the Primary Care Behavioral Health (PCBH) model, the BHC provides same-day consultations, brief interventions, and support for patients whose behavioral health needs impact their overall health and well-being. This role is ideal for a clinician who values team-based care, enjoys collaborating with medical providers, and is comfortable providing focused, solution-oriented care within the primary care visit structure. Position Overview: As a Licensed Mental Health Professional, you will work collaboratively as part of a multidisciplinary Primary Care team to address the mental and behavioral health needs of patients. Your role will include: * Provide same-day behavioral health consultations during primary care visits. * Conduct brief functional assessments of behavioral health concerns. * Deliver brief, evidence-based interventions (typically 1-6 visits). * Address common primary care behavioral health needs including: * Depression * Anxiety * Stress * Substance use * Chronic disease self-management * Sleep concerns * Health behavior change (medication adherence, diet, exercise) * Support patients with chronic medical conditions impacted by behavioral health factors. * Provide crisis assessment and stabilization when needed. * Work closely with Primary Care Providers (PCPs), nurses, care managers, and other team members. * Participate in warm hand-offs and curbside consultations. * Provide real-time recommendations to medical providers. * Assist with behavioral health triage and care planning. * Document consultations and interventions in the electronic health record (EHR). * Maintain documentation that supports integrated, brief care delivery. * Track patient outcomes and participate in population health management. * Participate in team huddles and case consultation. * Provide education to medical staff on behavioral health topics. * Support development and maintenance of PCBH model fidelity.
