The Lead Care Management Coordinator will coordinate incoming office referrals and verify insurance coverage. This role involves handling incoming calls from patients, families, physicians, and disciplines. The coordinator ensures that all personnel are informed of patient hospitalizations and follows up with patients admitted to other hospitals. The Lead Care Management Coordinator will also coordinate small programs with limited budgets and impact. The role requires applying substantial knowledge and experience to perform a wide range of advanced activities. The coordinator will serve as a working supervisor for teams or work groups. Effective communication skills are essential for facilitating telephone communications among nursing, therapy, aides, social services, and physicians regarding patient and staff schedule changes. The coordinator will assist with staffing and scheduling activities, soliciting input from managers. Follow-up for hospitalized patients is a critical duty, ensuring subsequent agency referral.
