Responsibilities: * Conducts initial assessment of prospective residents, including off-site visits * Performs wellness assessments and develops individualized care plans based on the needs and preferences of each resident * Delivers health promotion services, offers disease prevention counseling, and provides self-care education to residents * Engages with residents and family members in a compassionate and impactful manner, creating a lasting and positive experience * Fosters communication by regularly engaging with physicians, pertinent healthcare providers, and family members * Hires, trains, schedules, and leads the clinical team in providing exceptional care experiences * Coordinates the Community's infection control program while maintaining state regulatory compliance * Develop and oversee the Community's medication management program * Contributes toward quality assurance and performance improvement initiatives * Complies with all rules and regulations regarding confidentiality and privacy of resident information
