Shifts available: Monday thru Friday; 7a-3p, 7a-5p, 7a-7p, on call M-Fr 5p-7a, Sat. & Sun 7a-7aTypes of Cases: General Surgery, OB/Gyn/GYO, Ortho, Urology, GI, Cath Lab, CVL, Endo, Ent, spines, Very little aortic surgery. No cardiac transplant. Minimal Thoracic volume., Ortho, General, GYN/GYO/OB, CABG/Valves/TAVR/Mitraclip/LAA ligation/EPSupervision or independent: 70% their own cases and 30% supervisionSpecific requirements/restrictions: Board Certified: Required, DO NOT ADVERTISE PAY RATES IN ANY FORMAT, MUST PROVIDE COPY OF DRIVERS LICENSE AT PRESENTATION, MUST BE AVAILBLE TO WORK EVERY WEEK OF THE MONTH, MUST HAVE 100% CLEAN BACKGROUND - THIS INCLUDES ANY CLINICAL, LEGAL OR PENDING CASES., MUST BE ABLE TO PROVIDE 24 MONTHS OF CASE LOGS WITHIN 5 DAYS OF BEING APPROVED FOR CREDENTIALING, No Providers who worked for Envision within past 12 months and that live within 75 miles of the site, No Providers who worked for this site or an affiliated site in the past 24 months, No Providers that worked at this site or any Envision site that is within 75 miles of this site, New Candidates that have never worked for Envision must live outside the 75 mile radius of this site., No Providers will be accepted who currently reside or have a primary residence in the counties of Broward, Palm Beach, or Dade County.
