Compensation: $27-$30/hour dependent on licensure/credential and experience*$2 Additional Bilingual Pay DifferentialHours: Full-Time 40 hours/week. Early evenings may be required.Location: In community-based settings. The ability to travel between settings is essential.Job Summary This is a new and innovative role supporting a first of its kind model for the Finger Lakes region. Our In Community SUD Clinic removes traditional barriers by bringing substance use services directly into trusted community spaces where people already feel safe and supported. This can include, but will not be limited to, shelters, housing programs and FQHC's. The Substance Use Disorder Counselor in this role will provide high quality clinical services on site in these community settings while remaining an active and connected member of our robust outpatient clinic and multidisciplinary team. You will conduct assessments, provide counseling and facilitate immediate access to care when someone expresses readiness. You will work closely with Peer Advocates who offer hope through lived experience and with medical and clinical colleagues who support health screenings, medications for substance use disorder and harm reduction resources. If you want to help shape a new way of delivering compassionate care in our region, this role offers the opportunity to make a real and lasting impact.Essential Duties and Responsibilities Provide substance use counseling and recovery support in diverse community settingsConduct clinical assessments and facilitate immediate access to treatment servicesCollaborate with Peer Advocates, medical providers and counselors to support holistic careOffer harm reduction resources and education tailored to client goalsProvide closed loop referrals to health, housing, and social service organizationsMaintain timely, accurate documentation in accordance with OASAS standardsParticipate in multidisciplinary case conferencing and collaborative care planningSupport transition planning and ongoing engagement following an episode of careMaintain confidentiality in compliance with HIPAA and 42 CFR Part 2Participate in supervision and professional development activitiesTravel frequently between community partner sitesOther duties as needed to support quality care and access Supervisory Responsibilities This position does not include supervisory responsibilities.Minimum Qualifications Bachelor's degree required, Master's degree preferredCASAC, CASAC-T or Qualified Health Professional (QHP) required (e.g., (LCSW, LMSW, LMHC, MHC-L...).Minimum five years counseling experience preferredExperience working with individuals with co-occurring needs or justice involvement strongly preferredValid driver's license and reliable transportation required Knowledge, Skills and Abilities Strong engagement and relationship building skills in nontraditional settingsExcellent written and verbal communication skillsAbility to navigate access barriers and advocate inside complex systemsProficient use of electronic health records and Microsoft Office programsStrong organizational and problem-solving skillsAbility to work independently and as part of a collaborative teamCommitment to confidentiality and compliance standardsWork Conditions Physical Requirements Frequent standing, walking and travel across community sitesAbility to lift and carry up to five poundsRegular use of phone, tablet, computer and standard office equipmentEnvironmental Requirements Work performed in varied community environments and occasional office settingsComfort working in shelters, partner locations and public community spacesDisclaimer: All requirements are subject to possible modification to reasonably accommodate individuals with disabilities.This job description in no way states or implies that these are the only duties to be performed by the employee(s) occupying this position.
