Primary Purpose and FunctionThe Discharge Planner reports directly to the Assistant Clinical Director and is responsible for client's case management and assessment: identification of specific needs and social service intervention or referralMinimum QualificationsMinimum of one year of full-time experience, or its part-time equivalent, working in a behavioral health setting.Experience in residential recovery program preferred.Proficient computer skills and ability to use various software programs.Successfully complete drug screen and comprehensive background check.Have a valid driver's license in good standing and auto insurance.Ability to multi-task in a fast-paced work environmentCurrent CPR/First AidEssential Functions and ResponsibilitiesProvides case management, social services, utilization review and discharge planningDetermining client needs based on diagnosis, prognosis and social support system/person information are included in the initial assessment process.Demonstrates professional responsibility in the role of Discharge PlannerProvides case management, social services, utilization review and discharge planningMaintains appropriate confidentiality when interacting with clients, families, and staff.Support and maintain required compliance with State/Federal Guidelines and standards.Meets with each client and document sessions with a progress note.Attends and participates in clinical meetings regarding client's case management and discharge updates.Reports clients progress status and concerns to Assistant Clinical Director in a timely manner.Provides information in response to client inquiries.Maintains clinical competency in discharge planning/case management. Utilizes work time appropriately to maximize productivity.Provides initial discharge planning assessment within 24 hours of admission.Completes the assessment within 72 hours of admission for each client. Completion of assessment includes interview with the clients, family or other caregivers, and may utilize chart information from previous residential recovery visits.Refers clients for discharge with appropriate identified services. Preparation may include communicating with the management, clients, family, physicians, therapists, nurses, intake coordinators, insurance companies, reviewers, etc.Physician orders and signatures on transfer sheets, financial verification, insurance approval and transfer permits may need to be obtained, and transportation arranged.Completes client insurance reviews and note review outcomes, contacts and requests for further review times for follow-up.Case Management of clients may include arrangement of placement, communicating with insurance companies, assisting in filling out forms, and updating client notes frequently.Completes retro- review to insurance companies as directed by the Assistant Clinical Director.Performs other related duties as assigned.
