In this role, you will provide comprehensive rehabilitation care for residents in skilled nursing and long-term care facilities, focusing on pain management, functional improvement, and quality of life. You will collaborate closely with primary care providers, therapists, and interdisciplinary teams to develop individualized care plans and support optimal patient outcomes.ResponsibilitiesConduct comprehensive evaluations of patients with functional impairments, disabilities, and chronic conditionsDiagnose and manage rehabilitation and pain-related conditionsDevelop and implement individualized rehabilitation care plans in collaboration with interdisciplinary teamsPrescribe and oversee physical, occupational, and other therapeutic interventionsPerform procedures within scope of practice (e.g., joint injections, pain management techniques) as applicableCommunicate treatment plans, progress, and recommendations to patients, families, and facility staffMonitor patient progress and adjust care plans to optimize outcomesEnsure timely follow-up on treatment plans and interventionsDocument clinical findings, care plans, and outcomes accurately in the medical recordMaintain compliance with HIPAA, infection control standards, and facility policiesCollaborate with physicians, advanced practice providers, therapists, and nursing staff to support coordinated careParticipate in quality improvement initiatives and support best practices in rehabilitation careTravel to partner skilled nursing and long-term care facilities as assignedRequirementsDoctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) from an accredited medical schoolCompletion of an ACGME-accredited residency in Physical Medicine & Rehabilitation (PM&R)Board-certified or board-eligible in Physical Medicine & RehabilitationActive and unrestricted Physician license in the state of practiceCurrent ACLS and BLS certifications preferredPrior physician experience in rehabilitation medicine, pain management, or related settings preferredExperience working in skilled nursing or long-term care settings is a plusStrong clinical assessment and functional evaluation skillsExcellent communication and interdisciplinary collaboration abilitiesExperience providing care to the geriatric populationAbility to travel to multiple facilities as neededBenefitsTotal Compensation: Net CollectionsPaid Time OffMalpractice InsuranceWhy Work With Us?Make a meaningful impact on the lives of seniorsWork in a collaborative, mission-driven environmentEnjoy work-life balance Equal Opportunity EmployerInfinite Medical P.C is an equal-opportunity employer.
