Job Description

Practices and promotes the agency’s mission statement and promotes the agency’s guiding principles in every interaction. Provide ongoing support to people in becoming active participants in every aspect of their lives (i.e., self-care routines, health, safety, household chores, socialization, and overall wellbeing) and to ensurethat a high quality of life is maintained.Primary duties and responsibilities include but are not limited to: Ensures a safe and clean home by providing support to people to assist them with the completion of household chores and by participating in said household chores as neededFollows all treatment plans and utilizes all corresponding adaptive and safety equipment needed for each person to promote independence and ensure their health, safety and wellbeing.Supports people in preparing meals by teaching concepts such as diet adherence, portion control and cooking skillsAssists people with medication administration according to the AMAP policy and proceduresWorks with residential nurses to ensure accurate and current medical documentation for people supported if maintained.Implements and documents all treatment plans and other agency related materials (i.e., residential habilitation goals, medical case notes, communication logs, etc.) to assist people in achieving independence in their selected valued outcomes and to ensure their health, safety and well-beingAdheres to overtime policy and procedures to ensure that quality support is provided to people each dayAttends all necessary meetings to discuss each person’s progress and goal achievement and offer and gather information and ideasProvides transportation as needed for emergency situationsMentors new Direct Support Staff by demonstrating supportive routines and by reviewing training manuals to ensure new employees are sufficiently groomed to complete responsibilities.Responsible for documenting sleep checks through the night for the people supportedResponsible for knowing and participating in all emergency evacuation proceduresPerforms all other job-related duties as requested by supervisor.This position requires regular and substantial unsupervised or unrestricted physical contact with people receiving services and, therefore, is subject to successful completion of fingerprint/criminal background check High school diploma or equivalent preferred. Qualified NYS drivers' license required and maintained throughout employment. Ability to communicate and understand written and oral directives/information. AMAP Training is required and must be completed within six (6) months of hire. First Aid/CPR/SCIP-R Training is preferred.
