The MDS Coordinator Nurse is an RN or LPN who is responsible for the overall supervision, coordination, maintenance and timely completion of the MDS, care plans and routine assessments for residents. The Educator will conduct in person training for Chestnut Green staff as well as monitoring and enforcing Relias compliance for Chestnut Green staff. Primary Responsibilities: Essential Job Duties: 1. Coordinates, conducts, and completes the MDS in the Electronic Medical Record system. 2. Assures accuracy and completeness of the MDS. 3. Assures and monitors that MDS's are completed within state and federal guidelines. 4. Ensures MDS electronic submission occurs per State regulations. 5. Provides education to staff members including nurses and nursing assistants as needed regarding the documentation needed for accurate and complete MDS. 6. Assures effective communication with other shifts and departments as it relates to medical record documentation for the MDS completion to include completeness and accuracy of said documentation. 7. Communicates PDPM scores with accounting department to ensure accurate billing and reimbursement. 8. Communicates with the IDT team care plan items triggered from the MDS as well as any alerts from the MDS to include changes in condition and items that effect the residents care and the CMS Five Star rating for the SNF prior to transmitting the MDS. 9. Monitor Relias training to ensure compliance for all Chestnut Green staff. 10. Conduct as needed education sessions and ensure that all applicable staff complete the training. Other Duties: 1. Serves as a staff nurse when staffing levels necessitate in order to meet the needs of the residents. 2. Responds to emergency calls as necessary. 3. Assists DON in developing and implementing medical records policies and procedures. 4. Attends in-service training and education sessions, as assigned. 5. Performs specific work duties and responsibilities assigned by supervisor. 6. Participate in the Chestnut Green morning meeting and the IDT Clinical meeting each day. 7. Participate in the monthly Quality Assurance meeting by preparing and presenting the facility Quality Indicator and Quality Measure Reports. 8. Participates in the nurse manager weekend on call program. QUALIFICATIONS: RN or LPN currently licensed by the State of Maryland. Current CPR certification. Experience and knowledge regarding the MDS and MDS transmittal process and understanding of regulations impacting the MDS is preferred. Supervisory experience preferred. All new hires must be able to pass our preemployment screening process, which includes a fitness for duty physical, criminal background check, drug test and employment references as required by a licensed residential care facility.
