Join a team that prioritizes growth, open communication, and the tools you need to make a meaningful impact in the lives of others.Location: Sacramento Job SummaryThe Physical Therapist (PT) is a qualified professional who directs, supervises, evaluates, and provides physical therapy services to patients/clients in the home as prescribed by the attending physician.ResponsibilitiesProviding treatment as ordered by the attending physician, dentist, or podiatrist.Assisting in developing and updating the plan of treatment or Plan of Care (POC).Assisting the physician in evaluating level of function.Observing, recording, and reporting information on the patient's condition to the attending physician, case manager, and in the patient's health record.Advising, consulting, and when appropriate, instructing family and other LORIAN personnel in patient's therapy program.Teaching and supervising other healthcare personnel when appropriate.Evaluating the home environment and making appropriate recommendations.Participating in in-service education programs.QualificationsMinimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.
