Job Title: MDFT Therapist Location:Hempstead, NYJob Description: Position Summary: The MDFT Therapist provides intensive, home- and community-based clinical services to youth and their families. The therapist maintains a caseload of up to 10 clients and delivers services in alignment with the MDFT model to promote behavioral change, strengthen family functioning, and reduce justice system involvement.Key Responsibilities: Develop comprehensive clinical treatment plans based on assessments and intake evaluations. Provide individual and family therapy utilizing the MDFT model. Deliver trauma-informed care in home, community, and office-based settings. Participate in interdisciplinary team meetings and case reviews. Attend required trainings and ongoing professional development in MDFT and other evidence-based practices. Complete documentation and electronic medical records accurately and in a timely manner. Perform additional duties as required to meet client, agency, and regulatory standards. Minimum Qualifications: LMSW, LCSW, LMHC, or LMFT (Required). Minimum of 1 year experience working with children and/or adolescents. Bilingual in English and Spanish (Required). Valid NYS Driver's License in good standing and access to a reliable vehicle. Proficiency in electronic health records and Microsoft Office. Physical Requirements: Ability to read and write reports and case documentation. Ability to walk, stand, and sit for extended periods. Ability to bend, stoop, kneel, squat, climb stairs, and reach overhead. Ability to perform repetitive wrist and finger movements.
