If you've been looking for a role where clinical mastery, team building, and strategic influence come together with purpose, this is it.Position OverviewThe State Medical Director is a wound management certified physician leader responsible for clinical oversight, quality performance, and clinical excellence across the state. Reporting directly to the Chief Medical Officer, this role ensures the highest standards of wound care delivery within our in-home physician practice model.This leader combines deep wound care expertise with strong operational discipline, hands-on mentorship of newer physicians and mid-level practitioners, and direct clinical engagement. The State Medical Director maintains active involvement in patient care while driving statewide performance, provider development, and health plan collaboration.This is a working physician leadership role-grounded in the field, driven by outcomes, and accountable for measurable results.Key ResponsibilitiesDirect Patient Care Delivery (40%)Deliver wound care services both in-home and through a proprietary audio-visual platform for synchronous remote supervision and real-time clinical collaboration with Nurse Practitioners.Provide hands-on evaluation, mentoring, and evidence-based treatment planning for complex wounds (venous ulcers, pressure injuries, diabetic foot ulcers, post-surgical wounds, and other chronic wounds).Conduct structured ride-alongs with field clinicians, modeling clinical excellence, documentation integrity, and patient-centered communication.Clinical Leadership & Field Mentorship (30%)Provide clinical oversight for all wound care services delivered within the state, including regular chart reviews, case audits, and peer reviews.Serve as the clinical escalation point for Nurse Practitioners and physicians, delivering structured feedback and coaching focused on mastery of wound fundamentals.Lead recruitment, onboarding, performance management, and provider development while fostering a culture of high trust, integrity, and accountability.Health Plan Engagement & KPI Accountability (20%)Partner with executive leadership to manage health plan and health system relationships, serving as a credible clinical voice in value-based care discussions.Monitor, analyze, and report on key performance indicators including healing rates, time to wound closure, hospitalizations, infection, amputation, and utilization metrics.Drive measurable improvement through corrective action plans, performance reviews, clinical rounds, and strategic payer meetings. Organizational Leadership & Strategy (10%)Collaborate with the Chief Medical Officer on statewide growth strategy, protocol development, and clinical education programming.Help refine scalable systems that deliver high-quality care efficiently, with periodic travel to Tennessee for training, leadership development, and quarterly strategic meetings.RequirementsRequired QualificationsMD or DO degree from an accredited institutionActive, unrestricted Texas medical licenseWound care certification (CWSP, ABWM, or equivalent advanced wound credential)Demonstrated experience managing or supervising mid-level practitionersStrong background in complex and at risk wound managementExperience in value-based care and working with health plansA minimum of 5 years' experience delivering care in a variety of clinical settings Willingness and ability to travel regularly within the statePreferredExperience in in-home or community-based physician practice modelsMulti-site or multi-region medical leadership experienceExperience presenting clinical performance data to executive or payer audiencesWork Environment & TravelActive clinical engagement via in-home visits for training, mentoring, and consultation as well as synchronous remote supervisionRegular in-state travel for ride-alongs and direct oversightPeriodic travel to Tennessee for leadership meetings and trainingBlend of field-based, remote, and executive leadership responsibilitiesBenefitsHealth Care Plan (Medical, Dental & Vision)Retirement Plan (401k)Life InsurancePaid Time OffShort Term & Long Term DisabilityTraining & Development
