Job Title

Salary: $60-80+/hr based on experience LaSante Health Center is seeking an Optometrist to provide comprehensive eye care services in a federally qualified health center (FQHC) setting on per diem basis. This per diem role focuses on delivering high-quality vision care while collaborating with primary care and specialty teams to support overall patient health. Responsibilities: * Perform comprehensive eye examinations and vision assessments * Diagnose and manage common eye conditions and vision disorders * Prescribe corrective lenses and contact lenses as appropriate * Identify and refer patients with complex or surgical needs to specialists * Educate patients on eye health, preventive care, and treatment plans * Document all encounters accurately in the electronic health record (EHR) Qualifications: * Doctor of Optometry (OD) degree from an accredited program * Current New York State optometry license (or eligibility) * Experience in outpatient or community health settings preferred * Strong communication and patient engagement skills
