What You Will Do: Conduct comprehensive screenings and assessments of patients with cognitive and/or ADL physical functioning disorders.Develop individualized care plans aligned with regulatory standards and clinical best practices.Deliver skilled interventions, including ADL retraining, therapeutic exercises, modalities, and support device utilization.Monitor patient progress, modify treatments as needed, and evaluate outcomes to ensure optimal recovery.Provide counseling and consultation to patients, families, and caregivers about treatment goals and progress.Maintain accurate, detailed documentation of all treatments, assessments, and discharge summaries in electronic health records.Participate in team meetings, care conferences, and ongoing training.Support and supervise occupational therapy assistants, rehabilitation aides, and students, ensuring high-quality care delivery.Required Skills: Valid OT license and NBCOT certification.Proven experience with patient assessments, treatment planning, and documentation.Strong knowledge of regulatory requirements and clinical best practices in long-term care.Excellent communication skills for coordinating with interdisciplinary teams, patients, and families.Ability to adapt treatment plans based on patient progress and feedback.Detail-oriented, organized, and committed to compliance and ethical standards.Nice to Have Skills: Experience with geriatric populations and chronic conditions.Familiarity with electronic medical records and documentation systems.Additional certification in therapeutic modalities or specialized clinical skills.Preferred Education and Experience: Bachelor s or Master s degree in Occupational Therapy from an accredited program.At least 1-2 years of clinical experience, ideally within long-term care or rehabilitation settings.Other Requirements: Willingness to work Dayshift hours starting on May 11, 2026.Ability to travel locally for supplies or consultations if needed.Flexibility to adapt to facility-specific protocols and procedures.Valid state licensure and compliance with all professional ethical standards.Compensation for this role is competitive, with an hourly pay rate of $56.95.
