Physical Therapist Home Health San Jose, CA WellTech Partners is partnering exclusively with a five-star, clinician-led home health organization in the Bay Area and they're looking for a Physical Therapist who believes great outcomes start with genuine relationships.This isn't a revolving-door caseload. It's a role built for a clinician who wants to practice at the top of their license, own their day, and see the direct impact of their work one patient at a time, right in their home. Compensation & Schedule Salary: $130,000 $140,000 annuallySchedule: Monday Friday, 9 am 5 pmVisits per day: Approximately 5Benefits: Comprehensive package including health, PTO, mileage reimbursement, and more What You'll Be Doing You'll serve as a key partner in helping patients restore mobility, reduce fall risk, and regain independence all from the comfort of their homes. Working across San Jose and surrounding areas, you'll support a geriatric population with diverse clinical needs, bringing your expertise directly to the people who need it most.This is a field-based role where no two days look exactly alike and where your clinical judgment genuinely shapes patient outcomes.
