Job Description

Your role in this new full-time permanent career will be to support patients at home who are suffering from Chronic Kidney Disease (CKD) and End Stage Renal Disease (ESRD) by forming deep-rooted relationships and preparing them both emotionally and physically for the challenges of managing kidney disease. Experience with Palliative or dialysis patient care is a bonus. Expect a competitive salary starting at $110k and a 10% bonus based on experience.Requirements:Fully vaccinatedAbility to work Full Time, Mon-Fri 8 AM - 5 PM2+ years of NP experience in home health care or hospice2+ years of managed care experienceActive and unrestricted Nurse Practitioner licenseCurrent and unrestricted DEA certificate/licenseAbility to work without direct in-person supervision and practice autonomouslyHeavy daily travel; access to transportation, a valid driver's license, and car insurance.2-hour travel radius80%+ of the job is on the road visiting patients.Experience with computer-based data management programsExperience conducting annual wellness visits or similar comprehensive visits and appropriately charting HCCs; general knowledge of risk adjustment.Preferred Skills (Not Required):Experience in kidney disease and control of related co-morbidities (HTN, Diabetes, CHF) preferredExperience in end-of-life discussions, palliative care, or hospice.Experience practicing in managed care, and value-based context.Experience conducting annual wellness visits, comprehensive visits and appropriately charting HCCs.General knowledge of risk adjustment.ResponsibilitiesDeliver evidence-based, timely care in a manner that reduces hospitalizations, maximizes the quality of life, and puts the patient's health and satisfaction first.Conducts assessments, including comprehensive annual wellness exams.Prescribe medications, order tests, and collaborate with physicians.Collaborate with care management RNs and Social Workers to ensure consistent highly satisfying patient experience, accurate documentation, and effective care planning.Perform effectively, as reflected by improved patient quality outcomes, which will be measured and reported daily.Opportunity to work in a dynamic, fast-paced, and innovative care management company that is transforming the delivery of kidney careComprehensive medical, dental, vision, and life insuranceFlexible paid leave & vacation policy401(k) plan with matching contributionsHealth Savings Account (HSA) and Flexible Spending Account (FSA)Depending on travel times, NP may only have 1 or 2 patients per day but are compensated according to a full-time salary!Interview ProcessScreening Phone Interview with an advisor for the companyZoom Video interview with the Operations Manager #J-18808-Ljbffr
