You ll work closely with an interdisciplinary team (IDT) to assess, plan, and manage care for a medically complex, primarily geriatric population focusing on continuity, prevention, and whole-person care rather than episodic treatment. In this role, you ll manage participant care plans, oversee medications, coordinate with specialists, and support care transitions across settings. You ll play a key role in daily IDT collaboration, contribute to quality improvement initiatives, and help guide clinical decision-making that prioritizes appropriate utilization of resources.
