This role focuses on managing complex medical needs for an aging population through comprehensive assessments, care planning, and close collaboration with an interdisciplinary team.The ideal candidate is skilled in primary care, values continuity of care, and thrives in a collaborative environment. Responsibilities include conducting thorough evaluations, managing chronic and acute conditions, coordinating care across settings, and supporting safe transitions.
