Position Description The Cardiology Clinic Advanced Practice Provider provides clinic support services to providers and patients at the 999 Adams Street Adventist Heart and Vascular Clinic.Location / Work HoursPosition is based on site at the Adventist Cardiology Clinic, 999 Adams Street, St. Helena California.12 hour inpatient shiftsPosition QualificationsAPP is responsible and accountable for safe clinical practice; must be knowledgeable of cardiac, vascular, and cardiothoracic patients, medications, standard treatments. Maintain knowledge of current cardiac medications and procedures.Minimum of three years of recent RN/NP/PA clinical experience in acute patient care, ICU or ER.Outstanding communication and telephone skillsAbility to communicate via telephone with patients and provide good customer serviceDemonstrates professional oral and written communicationMakes decisions that adhere to policy and use critical thinking skillsPossesses ability to effectively manage timeJob ResponsibilitiesProvider SupportClinical management of coumadin patients for St. Helena Napa Valley region clinics including 999 Adams clinic, Woodland St. Helena clinic, and Vallejo clinics. See patients in clinic for education and management. Maintain and update Coumadin clinic tracking system.Perform Cerner order proposals and medication refill proposals per provider guidelines.Call patients of lab/test/procedure results within 24-48 hours of completion, including Zio Patch Monitoring, EKGs.Communicate effectively lab and test normal and abnormal values or results and the importance.Supervise exercise and nuclear stress tests per Adventist Health standard protocols.Perform peer-to-peer authorizations with insurance companies for tests/procedures. Complete patient-required forms including return to work letters, employment forms, and disability forms. Perform cardiac clearances required for patients undergoing procedures and surgeries. Perform patient office visits for low acuity patients: blood pressure control, post cath lab procedures, coumadin education. As needed, perform hospital discharges post- cath procedures.Telephonic Patient Support Provide cardiac advice, education, and guidance telephonically for cardiac patients. Triage patient phone calls and advise patient on proper course of action in accordance with standards of care and established protocols/guidelines or in consultation with provider. Return patient phone calls in a timely manner - no later than the next business day, depending on urgency. Evaluate emergency patient situations and initiate appropriate patient care actions. Assess patient symptoms, determine acuity, identify appropriate action needed, advise patient appropriately. Document in Cerner and notify providers if urgent. Provide referrals to healthcare providers and community resources such as smoking cessation when appropriate.Provide patient education and counseling on lifestyle modification, medications, and other areas as needed. Provide appropriate medical and nursing advice based on knowledge and/or what the provider specifies. Document in Cerner each patient telephone call and information provided to patient. Perform service recovery calls and document issues to improve clinic experience.Clinic Staff Training and Support Ability to be flexible and available to help clinic staff as needed. When needed, assist provider with exams or procedures.
