Summary The Manhattan VA Health Care System is seeking a full-time Nurse Manager / Patient Care Team Coordinator. Responsibilities The Registered Nurse - MICU PCTC Manager is accountable for a 12 bed ICU and 6 beds CCU, a Cardiac Cath Lab which has 2 monitored rooms for invasive Cardiac procedures. Also responsible for overseeing the TAVR program. Patient Care Team Coordinator (PCTC) is accountable for the management, supervision and evaluation of patient care delivered by both nursing and allied health staff (over 50 FTEE's) on a 24/7 basis. The PCTC of MICU/CCU and Cardiac Cath Lab is responsible for the clinical coordination of patient care to achieve quality nursing care for optimal patient outcomes. The PCTC is accountable for the management, supervision and evaluation of care delivered by nursing and other allied health staff for assigned areas on a 24-hour basis. As leader of this interdisciplinary team, the PCTC uses appropriate consultation with experts of each discipline he/she supervises to ensure that practice and performance issues are addressed and resolved General Description of Assigned Duties: Functions as a clinical expert and is responsible and accountable for all elements of the nursing process when providing or supervising direct patient care. Assesses, plans, implements, and evaluates delivery of care based on age specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. They demonstrate sound decision-making and judgment and possess excellent interpersonal skills. They are self-directed and influences care outcomes by collaborating with members of the interdisciplinary team and leading innovative process improvement initiatives. Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care.Functions or Scope of Assigned Duties: The Nurse Manager (NM) is the designated first-line supervisory authority and is responsible and accountable for the 24/7 operation of their assigned units and provides coverage for other NMs as needed. The NM is responsible and accountable for the direction and supervision of specific orientation, training, development of staff, and evaluation of individual staff performance. They function as a role model, educator, practitioner, and supervisor providing leadership to the team. They influence care outcomes by collaborating with members of the interdisciplinary team to remove barriers and implement new evidence based cutting-edge programs/processes. Preferred experience: 3-5 years of Acute care ICU RN experience and/or Cardiac Cath Lab RN experience and MSN and 2-3 years progressive leadership/Supervisory as a Nurse Manager in a major medical Center This position is not approved for telework and staff is required to be on onsite in the clinical area of assignment. This is a VA-NYHHCS Position. This position supports all 3 campuses and may require rotation as needed.
