RESPONSIBILITIES I. JOB SUMMARY/RESPONSIBILITIES: u2022 Develops and implements a progressive rehabilitation program for patients in their homes in accordance with the principles and practices of physical therapy and the clientu2019s needs. u2022 Serves as case manager and supervises home health aides and physical therapist assistants when appropriate. u2022 Performs duties in accordance with policies and procedures as established by the agency. II. TYPICAL PHYSICAL DEMANDS: u2022 Sitting, finger dexterity, seeing, hearing, speaking. u2022 Constant: lifting maximum weight of 50 pounds, carrying usual weight of 20 pounds, pushing/pulling maximum weight of 50 pounds of force. reaching above, at and below shoulders; repetitive arm/hand motion and frequent gripping of objects. u2022 Frequent: standing, walking, stooping/bending, and twisting body. III. TYPICAL WORKING CONDITIONS: u2022 Subjected to adverse environmental conditions like blood borne pathogens. IV. MINIMUM QUALIFICATIONS: A. EDUCATION/CERTIFICATION AND LICENSURE: u2022 Current Hawaii State license as a Physical Therapist. u2022 Masteru2019s Degree or Doctorate Degree in Physical Therapy. u2022 Valid U.S driveru2019s license, vehicle and vehicle insurance as required by Hawaii State Law. u2022 Driveru2019s abstract or evidence of good driving record. u2022 Current BLS certification. B. EXPERIENCE: u2022 Knowledge of occupational therapy theory and practice to treat patients. u2022 Knowledge and use of medical equipment and instruments to administer patient care. u2022 Skill in identifying problems and recommending solutions. u2022 Skill in establishing and maintaining effective working relationships with clients, co-workers, and the public. u2022 Ability to communicate in English effectively both orally and in writing. u2022 Ability to interpret, adopt, and apply guidelines and procedures. u2022 Ability to react calmly and effectively in emergency situations.
