The HCC provides in-home case management to PACE participants and provides oversight to home care aides to ensure that the participants in-home plan of care is effectuated.ResponsibilitiesAssesses in-home care needs by doing an in-person assessment in the participant's home on admission and routinely thereafterCoordinates appropriate care with internal and external care providers (home care aides)Provides oversight to the plan of care by observing care and reviewing recordsCoordinates with compliance to ensure that caregivers are following the plan of careAssess for change of condition and completes new assessments as neededProvides case management for in-home care needs for supplies/equipment and supports changes to plan of care when there is a change in participant status, i.e. acute illness or injuryCoordinates with the Inter-Disciplinary Team to ensure that the established plan of care is effectuatedRequirementsTwo years of experience in case management or care coordination in healthcare setting or community agencyAdvanced degree in health field preferred i.e. MSWLicense in health field preferred i.e. PTA, COTA, OT, PT, LCSW, LPN, RNOur ValuesAs a MultiCare employee, we'll rely on you to reflect our core values of Respect, Integrity, Stewardship, Excellence, Collaboration and Kindness.
