Job Description

Substance abuse assessment, treatment and referrals will be provided to those recently released from state prisons through theDay Reporting Center in the AlabamaPardons and Parole office. Former inmates come typically everyday to receive services such as GED preparation, substance abuse and mental health treatment, or assistance with employment.Provide effective clinical evaluation in completing the Human Services Needs AssessmentFormulate appropriate Case Plans along with the consumer to document each goal/problem and how it will be addressed within one week of completing the HSNAProvide advocacy and referrals to community resources to assist consumers in advancing towards recovery and re-entryProvide didactic groups designed to teach skills and monitor consumer progress in recovery and re-entryProvide monitoring services to consumers towards progress/achievement of goals.Effectively collaborates with team members and other service providers in the delivery of clinical services.Assist in evaluating consumers for discharge planningEffectively interacts and corresponds with Alabama pardons and parole staffAdheres to professional code of ethicsWill work closely with the treatment team to identify needed educational/skill development groupsWill provide services, including groups, to meet identified needs of consumers (budgeting, parenting, job interview skills, etc.)Will utilize the Case Management sign-out sheetWill offer/provide GED tutoring to consumersWill provide services to consumers whether they are active in group or notWill involve community resources to provide sober, social or educational activitiesWill model good time management for consumers by being on time for workWill document progress notes in such a way as to reflect the consumer’s treatment goals.Will complete the Case Management enrollment on the same day as the Case PlanSupervision and consultation.Seeks clinical supervision and consultation as neededAccepts and employs suggestions for improvementActively works to enhance clinical skillsClinical record keeping.Document services provided in a timely manner per AltaPointe policyMaintain current weekly progress notes, group notes, and weekly summary reports system.Effectively manage caseload based on consumer’s needsMeet AltaPointe Health System productivity standardsBachelor degree in a mental health discipline; certification of substance abuse (SA) case management training by Alabama Department of Mental Health (DMH); knowledge in substance abuse and mental illness specialties, outlined below, to be obtained within six (6) months of employment:Knowledge of the nature of addiction and psychiatric disordersKnowledge of gender specific issues relative to addictionKnowledge of substance abuse treatment principlesKnowledge of the 12-step support groups such as AA, NA, CA, and other self-help groups for co-occurring disordered adultsKnowledge of criminogenic needs, definition and risk factorsUnderstand the dynamics of co-dependency of addicted individuals, denial, manipulation and other typical behaviors of these consumersUnderstand the signs and symptoms of active mental illness and has the information and skills to address crisis intervention or intervention needsUnderstand the behavioral, psychological, physical health, and social effects of psychoactive drugs, including alcohol and tobacco, on the consumer and significant othersKnowledge of helping strategies for reducing the negative effects of substance abuse dependency and how to tailor strategies and treatment modalities to the consumers’ stages of dependency, change or recoveryKnowledge of medical and pharmaceutical resources in the treatment of addictive and mental illness diseasesKnowledge of cultural diversityKnowledge of the risk for illnesses among IV drug users such as HIV/AIDS and T.B.Knowledge of the criminal justice system including court hearings, judges, court referral officers, probation officers, and jail diversionKnowledge of HIV/AIDS education, counseling and testingKnowledge of behavioral management and crisis intervention
