Cardiology Nurse Coordinator is a registered nurse who is independently accountable for the effective management, coordination of care, and development of education for specific patient populations being evaluated for advanced cardiac disease, procedures and treatments. The Coordinator partners with the general cardiologist, interventional cardiologists, advanced practice practitioners, and other providers and clinical staff throughout the ARH system, inpatient and outpatient departments, local and regional healthcare facilities to coordinate patient care. In collaboration with the multidisciplinary team, the Coordinator delivers patient and family centered care that meets or exceeds the ARH standards of quality care in the Cardiac Service Line. The Coordinator has a strong knowledge base and nursing expertise in cardiovascular diseases and treatment modalities. After consulting with the multidisciplinary providers, the Coordinator communicates with local and regional referring physician providers and clinical staff to ensure continuity of patient care.Conduct interviews with patient, family members and heath care teams Educate patient and family regarding all aspects of disease management, diagnostic and interventional proceduresDevelop nursing care plans to ensure standards of care are achievedReview patient schedules proactively to ensure efficiency in clinical workflowsEnsure appropriate resources are available to support patient care and procedures Educate patient and family of signs and symptoms that require a follow up telephone call or triage visitCollaborate with medical faculty, nursing personnel and other health care agencies to facilitate patient care needsCommunicate and solve problems with multidisciplinary teams to solve complex patient care issuesAssist with accurate data collection and analysis for process improvement and quality initiative Serve as a resource person to staff, referring providers, and clinical staff Attends in-service and continuing educationRespects the dignity and confidentiality of all matters related to the patientsPerforms other related duties as required within scope of the job classificationEducationGraduate of an accredited school of nursingRegistered Nurse, Licensed by Kentucky State Board of NursingMinimum Work ExperienceFive (5) years clinical care setting (cardiovascular)Required Skills, Knowledge, and Abilities• Basic Life Support (BLS)• ACLS Certification • Demonstrate success with critical thinking and prioritization skills• Demonstrate flexibility and time management skills to meet program and patient care needs• Knowledge of quality improvement and standards of care within practice area• Experience in teaching patients and families• Excellent organizational skills• Ability to work independently
