Advanced Practice Provider (APP) u2013 Transplant (Solid Organ Transplant Recipients and Living Donors) The Transplant APP provides comprehensive care to liver, kidney, and pancreas transplant recipients and living donors. Working both independently and collaboratively with physicians and an interdisciplinary team, the APP performs detailed assessments, formulates diagnoses, orders and interprets diagnostic studies, and develops and manages treatment plans. Responsibilities include prescribing therapies, initiating specialty consultations, managing acute and emergent clinical situations, and facilitating efficient patient flow. The APP communicates care plans effectively with patients and families, provides specialty consultation, supports clinical pathways and protocols, and ensures patient advocacy, education, confidentiality, and high-quality, patient-centered care. This is a 24/7 position and a nocturnal option is available. Experience: New graduate will be considered, experienced clinicians also considered Background checks and pre-employment health screenings will be conducted on all new hires prior to employment. All safety-sensitive positions with direct patient interaction also require a drug screen. The Transplant Advanced Practice Provider (APP) delivers comprehensive, high-quality care to transplant recipients and living donors across the continuum of care. The APP practices independently and collaboratively with transplant physicians/ surgeons and interdisciplinary team members to assess, diagnose, treat, and manage complex transplant patients while supporting clinical operations, patient education, and interdisciplinary collaboration. Administrative Responsibilities u00b7 Ensures accurate, timely, and compliant documentation in the electronic medical record u00b7 Maintains confidentiality of patient information in accordance with institutional, state, and federal regulations u00b7 Facilitates timely patient flow through effective coordination with interdisciplinary team members u00b7 Supports clinical operations through adherence to institutional policies, procedures, and clinical pathways u00b7 Participates in quality improvement initiatives, clinical audits, and workflow optimization as assigned Professional Responsibilities u00b7 Practices in accordance with professional standards, regulatory requirements, and scope of practice for Advanced Practice Providers u00b7 Communicates effectively with patients, families, physicians, nursing staff, and interdisciplinary team members u00b7 Advocates for patients and families regarding educational needs, treatment options, ethical considerations, and patient rights u00b7 Demonstrates cultural competence and sensitivity to the diverse needs of transplant patients and families u00b7 Engages in ongoing professional development, continuing education, and maintenance of certification and licensure Clinical Responsibilities u00b7 Provides comprehensive clinical care to transplant patients independently and in collaboration with transplant physicians and interdisciplinary team members u00b7 Performs complete and focused histories and physical examinations u00b7 Documents clinical findings, including pertinent normal and abnormal findings, in the medical record u00b7 Orders, interprets, and evaluates diagnostic laboratory tests and imaging studies u00b7 Formulates medical diagnoses based on clinical assessment and diagnostic findings u00b7 Develops, implements, and evaluates individualized treatment plans u00b7 Prescribes and recommends pharmacologic and non-pharmacologic therapies within scope of practice u00b7 Initiates specialty consultations and referrals as clinically indicated u00b7 Recognizes and responds to acute and emergent clinical situations, initiating life-saving interventions and activating appropriate interdisciplinary support u00b7 Provides specialty consultation related to abdominal transplant care u00b7 Explains the necessity, preparation, nature, and anticipated outcomes of procedures to patients, families, staff, and learners Leadership Responsibilities u00b7 Collaborates with interdisciplinary team members to initiate, evaluate, and refine clinical pathways and protocols u00b7 Supports and strengthens interdisciplinary relationships to promote coordinated, patient-centered care u00b7 Serves as a clinical resource and role model for staff, students, and trainees within the transplant program u00b7 Contributes to the advancement of transplant care through participation in program development, quality initiatives, and best-practice implementation u00b7 Promotes a culture of safety, accountability, and excellence in transplant patient care MINIMUM REQUIREMENTS Education: Graduate of an accredited graduate school of nursing or physician assistant program with a masteru2019s degree. Experience: Department and unit specific. Licensure: License or license eligible as an Advance Practice Provider (Nurse Practitioner, Physician Associate, Certified Registered Nurse Anesthetist, and Certified Nurse Midwife) as set forth by the state of Virginia. If Nurse Practitioner, license to Practice as a Registered Nurse in the Commonwealth of Virginia is required. Must demonstrate and maintain all credentialing and licensing requirements of UVA Health System, the Virginia Board of Medicine or the Virginia Board of Nursing, and appropriate certifying bodies. Current Basic Life Support (BLS) required. Current PALS/NRP, as required, specific to position. Advanced Cardiac Life Support (ACLS) as required, specific to position. PHYSICAL DEMANDS Job requires standing for prolonged periods, frequently bending/stooping, reaching (overhead, extensive, and repetitive); Repetitive motion: computer keyboard. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 50 - 100lbs. May be exposed to noise, radiation, radioactive materials, blood/body fluids and infectious disease. The starting base rate for this role is $129,323.00 annually. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education.
