Delivers quality home health care services to the Agency’s clients under the direction of a Registered Nurse in accordance with physician orders, Agency and industry standards, and federal and state regulations.Assists members with activities of daily living, including bathing, dressing, grooming, toileting, eating, transferring, and mobilityEngages members in meaningful conversation, socialization, and activity while providing personal care assistanceResponds to all members needs and/or requests for care promptly, and provides or obtains assistance with care as necessaryObserves members for changes in physical, emotional, mental, or behavioral condition, unusual symptoms, accidents, injuries, or unusual occurrences, and reports promptly to a licensed nurseCompletes all forms and documentation such as incident reports, change of condition reports, and behavior tracking, as requested, and in a timely mannerPrepares and serves meals, assists with eating, not including tube feedingAssist with self-administration of medicationsMake and change bed linensClean immediate living areas including bathroom and bedroom and remove trashEducation and Experience:•CPR Certificate (preferred)• High school diploma or equivalent• Previous experience working with the elderly in a residential or long-term care setting also preferred
