Adecco Healthcare & Life Sciences is assisting our client hire an RN u2013 Case Manager in St. Cloud, Florida This role is in person/onsite. Schedule: Monday u2013 Friday, 8am to 5pm Pay: $39-$ 46.50 Responsibilities of the RN-Case Manager + Assist in reducing overall admissions, readmissions, and ER visits + Support patients with highu2011risk health conditions in navigating the healthcare system + Promote patient empowerment by serving as an educator, resource, and advocate for patients and families + Collaborate with multidisciplinary care teams including physicians, nurses, pharmacists, lab technologists, social workers, and educators + Coordinate care plans and ensure continuity of care + Monitor patient progress and maintain accurate, timely documentation + Communicate clinical updates and provide patient/family education + Perform case management follow-ups, care plan updates, and related duties + Work independently with strong organizational and timeu2011management skills + Utilize MS Office, Excel, and electronic systems as required + Complete other related duties as assigned Qualifications of the RN + Active and unrestricted Registered Nurse (RN) license in Florida + BLS certification + Healthcare professional experience in a hospital, urgent care, or primary care setting + Excellent verbal and written communication skills + Strong customer service and interpersonal skills + Ability to work in a fastu2011paced environment and multitask + Working knowledge of medical terminology and HIPAA regulations + Ability to learn and maintain medical equipment + Proficiency with MS Office and Excel + Ability to complete a coloru2011blindness test + _(Medicare Advantage experience preferred)_ Why work for Adecco? + Weekly Pay + 401(k) Plan + Skills Training + Excellent medical, dental, and vision benefits Benefit offerings include medical, dental, vision, life insurance, shortu2011term disability, additional voluntary benefits, EAP program, commuter benefits, and 401(k).
