We invite you to become part of Connecticut’s most comprehensive healthcare network as a Licensed Practical Nurse.For over 60 years, St. Vincent’s Special Needs Services (SVSNS) has been a trusted provider of specialized services for children and adults with multiple developmental disabilities and complex medical needs, such as cerebral palsy, acute brain injury, neuromuscular disorders and autism spectrum disorders. Each year, more than 400 children, adults and their families receive services from the organization.Summary:Provide school-based nursing and educational support to medically complex special need students. Ensure implementation of health care plans, promote student safety, and support access to learning through collaboration withschool staff and families.Key Responsibilities:Serve as primary caregiver for identified students during the school dayImplement individualized plans of care and emergency care plansMonitor student health and report changes to the RNAssist with medical emergenciesAdminister medications and treatments; complete accurate daily documentationAccompany students during field trips, and school activitiesCoordinate medical equipment and suppliesCommunicate effectively with families and interdisciplinary teamsParticipate in team meetingsAssist students during emergency drills Qualifications Current LPN license (required)Pediatric or school nursing experience preferredStrong communication and documentation skillsWe take great care of careers.With locations around the state, Hartford HealthCare offers exciting opportunities for career development and growth.
